Social Worker, MSW (Full-time 40hrs.)


Location: Bridgeport, CT
Exempt/Non-Exempt: Exempt

Summary: Provides casework and counseling services to Residents and their families.

Titles Supervised:

Shift: Full-time 40hrs.

Duties:
Essential Responsibilities:
1. Experienced with skilled nursing (LTC), assessing the needs of patients/residents and families, providing support and guidance, developing and coordinating discharge plans, referrals to appropriate agencies, MDS, Care Planning.

2. Facilitate discussion with resident/patient, responsible parties and staff regarding concerns. Facilitate meetings with responsible parties and support groups as necessary. Make notations in medical record. Maintain complaint log.

3. Participate in designing, and on occasion present, in-services to Mozaic Senior Life staff regarding issues related to resident rights and psychosocial needs of the elderly. Supervise graduate and undergraduate students in social work and/or volunteers as time permits.

4. Coordinate and complete investigation of any abuse allegations submitting written reports as requested.
5. Represent the resident/patient in court matters when necessary.
6. Other related duties as assigned.

Qualifications:
Minimum Qualifications:

Education: Master's Degree in Social Work preferred.

Experience:1-3 years clinical experience with the elderly population preferably in a LTC setting

Licensure: MSW preferred

Knowledge/Skills/Abilities:

Ability to handle multiple demands and deadlines; Ability to communicate effectively; Ability to utilize various software packages to carry out position responsibilities.
